The Substance Use Disorder (SUD) Reentry Program provides specialized support for individuals with substance use disorders and co-occuring mental health conditions as they transition from incarceration back into the community.

Case Management
Providing individualized support with housing, employment, healthcare, benefits, and other essential needs to help participants achieve stability and self-sufficiency.
Community Reintegration Services
Supporting successful reentry through probation compliance, life skills development, resource navigation, and positive community connection.
