The Substance Use Disorder (SUD) Reentry Program provides specialized support for individuals with substance use disorders and co-occuring mental health conditions as they transition from incarceration back into the community. 

Services Provided

Substance Use & Mental Health Support

Connecting participants with treatment, counseling, recovery resources, and peer support services that promote healing and long-term wellness.

Case Management 

Providing individualized support with housing, employment, healthcare, benefits, and other essential needs to help participants achieve stability and self-sufficiency.

Community Reintegration Services

​Supporting successful reentry through probation compliance, life skills development, resource navigation, and positive community connection.

​Eligibility Requirements

Participants must:

• Be on probation or parole
• Be within 120 days of community reentry, including individuals transitioning through supervised or early release programs
• Be experiencing substance use disorder (SUD) and/or co-occurring mental health challenges
